It is essential that all visits are re-confirmed within 24–48 hours of the scheduled date and time. Patient circumstances can change at any point between visits, and it is not sufficient to rely solely on confirmation obtained during the prior visit, even if that visit was just a week ago. Timely confirmation within the required timeframe allows for the identification of any issues that may prevent the visit from occurring, including but not limited to:
Changes in patient availability
Delays in receiving medications or supplies
Hospitalization
Insurance holds
We recognize that some circumstances are beyond our control and that, even with proper re-confirmation, there may still be occasions when a visit cannot take place as planned. Re-confirming visits within the required timeframe significantly helps reduce these situations by identifying potential barriers in advance and allowing adjustments to be made when possible, helping to prevent unnecessary travel to patients’ homes.
In an effort to minimize these “incomplete” visits, many of which cannot be billed to our vendors, we will be monitoring these occurrences more closely. We ask that everyone continue to do their part by ensuring visits are re-confirmed within 24–48 hours of the scheduled date and time.
Thank you so much for all you do!
Sincerely,
Shari Blackmon RN-C, BSN
