Support Ticket for Tracking Missing Supplies

We are continually looking for ways to improve efficiency and ensure the highest quality of care for the patients we serve. While we prioritize addressing issues within our immediate control, we also recognize the importance of identifying and addressing challenges that may seem beyond our direct influence.

 

In home care, our ability to provide safe and effective treatment is closely tied to the timely and accurate delivery of supplies from the Pharmacy. These supplies are essential for administering infusions and delivering comprehensive care in the home. When supplies are missing or incomplete upon arrival, it can place us in difficult and frustrating situations. We also acknowledge and deeply appreciate the resourcefulness many of you demonstrate in these instances, sometimes relying on your car stock to bridge the gap and ensure patient care is not compromised.

 

To support efforts in reducing these supply issues, we are asking for your help to assist us in tracking instances of missing or incomplete Pharmacy deliveries. This documentation will help us identify trends, particularly with specific Pharmacies or recurring items, and will strengthen our ability to engage in productive discussions with our vendors focused on process improvement.While we understand that occasional human error is inevitable, our goal is to identify and address recurring issues. We are hopeful that, through collaboration and shared accountability, we can achieve meaningful improvements in areas where consistent patterns/deficiencies are noted.

 

Our ultimate goal is to use this information constructively—to advocate for the resources you need and to minimize the likelihood of these situations occurring in the future.If you encounter a situation where essential supplies needed to complete an infusion are missing, please submit a Support Request Ticket. This ticket can be accessed via our HHC Resources page https://www.hhc-resources.com/ or through the Logiforms Nurse Portal. 

 

Please note: This process is intended for instances where critical supplies required to perform the infusion or provide care are not provided for tracking purposes. This does NOT take the place of the Supply Forms that you fill out in the home, send in with your visit documentation, and are then forwarded to the Pharmacy.
