HHC Forms:​ All Visit Notes & Other Forms (Folder Access Link)

HHC forms are generally available electronically via the Visit Note Portal. Electronic charting is the required method, unless electronic forms are not available, or device failure / cellular connection prevents electronic charting from occurring. Paper copies of HHC forms should be maintained in your supply kit for back up purposes.
