MIXING
Obtain a clear/clean space to prepare the AVLAYAH
Be sure all the necessary supplies are available to complete infusion from beginning to end. You will need:
IV start supplies, alcohol, infusion bag, 0.9% Sodium Chloride Injection bag, syringe and needles, medication vials, Sterile Water for reconstitution, tubing with micron filter, pump
Remove the required number of AVLAYAH vials from the refrigerator and set aside for 15 to 30 minutes to allow vials to reach room temperature
Reconstitute each vial with 5.2 mL of Sterile Water for Injection by slowly injecting the diluent onto the inside wall of each vial to avoid foaming.
Gently swirl each vial to completely dissolve the lyophilized powder, do not invert or shake the vial
Each reconstituted vial will yield a concentration of 30 mg/mL of tividenofusp alfa-eknm. 5)
Inspect reconstituted solution for particulate matter and discoloration, solution should be clear to slightly opalescent and colorless to slightly brown/yellow, and free of visible particles.
Withdraw a volume of 0.9% Sodium Chloride Injection from the infusion bag equivalent to the volume of AVLAYAH to be added
Withdraw required volume of reconstituted solution from AVLAYAH vial(s) and slowly inject into infusion bag
Discard unused portion after each use; do not administer more than one dose from the vial
Avoid introducing air into the infusion bag
Gently invert the infusion bag to mix the solution, do not shake.
ADMINISTRATION
Take baseline vitals and document basic assessment
Infuse AVLAYAH over approximately 4 hours per recommended infusion rates
Increase the initial infusion rate to the subsequent infusion rate every hour based on patient tolerance
Total infusion time should not exceed 8 hours
Monitor patient for any adverse reactions
Record vital signs at baseline, every 15 minutes for the first hour, hourly and with each rate change, AND 5-10 minutes post infusion
Record vitals every 15 minutes after infusion start, AND 5-10 minutes post infusion
Record lot numbers and expiration dates for vials used
At end of infusion, flush infusion line with 0.9% Sodium Chloride Injection using the final infusion rate that was used to administer AVLAYAH.
Disconnect patient and remove IV
Place pressure bandage at site
Clean and dispose of used supplies
