MIXING
Obtain a clear/clean space to prepare the CEREZYME.
Be sure all the necessary supplies are available to complete infusion from beginning to end. You will need: medication vials; syringes and needles for reconstitution; Sterile Water for Injection, USP; 0.9% Sodium Chloride Injection bag; tubing with a 0.2 micron-in-line filter; pump
Reconstitute each vial of CEREZYME by slowly injecting 10.2 mL of Sterile Water for Injection, down inside wall of each vial
Gently tilt vial to allow powder to dissolve completely
Dilute CEREZYME solution for infusion promptly with 0.9% Sodium Chloride Injection to final volume that is calculated based on prescribed dose
ADMINISTRATION
Take baseline vitals and document basic assessment
Spike the bag of diluted medication with tubing
Infuse at rate ordered by pharmacy
Infusion usually is given over 1-2 hours
Monitor patient for any adverse reactions
Record vitals every 15 minutes after infusion start, AND 5-10 minutes post infusion
Record lot numbers and expiration dates for vials used
Disconnect patient, flush line, and remove IV
Place pressure bandage at site
Clean and dispose of used supplies
