MIXING
NOTE: NURSE must stay for entire infusion.
Obtain a clear/clean space to prepare the DALVANCE.
Allow DALVANCE to reach room temperature before reconstitution.
Be sure all the necessary supplies are available to complete infusion from beginning to end.Â
You will need IV start kit, alcohol, sterile water or 5% Dextrose for each 500 mg vial, syringe and needle to reconstitute DALVANCE
Withdraw 25 ml of sterile water and add to the vial of DALVANCE.
Do not shake but gently swirl until completely dissolved.
500 mg vial reconstituted contains 20 mg/ml and is a clear, colorless to yellow solution.
Transfer the required dose of reconstituted DALVANCE solution for the vial(s) to an IV bag containing 5% Dextrose Injection, USP.
The diluted solutions must have a final Dalbavancin concentration of 1 mg/ml to 5 mg/ml.Â
IV line MUST be flushed BEFORE and AFTER each DALVANCE infusion with 5% Dextrose Injection, USP.
DO NOT co-infuse DALVANCE with other medications or electrolytes. Saline-based infusion solutions may cause precipitation and should not be used.
ADMINISTRATION
Take baseline vitals and document basic assessment.
Flush line with10 ml D5WÂ and then infuse at rate ordered by pharmacy.Â
Record vitals every 15 minutes for the first hour, then hourly AND then 5-10 minutes post infusion.
Record lot numbers and expiration dates for vials used.
Disconnect patient, flush line with 10 ml D5W and then 10 ml NS and remove IV.
Place pressure bandage at site.Â
Clean and dispose of used supplies.
