ALWAYS confirm your medication dose against the medication label and orders

ALWAYS reach out to us first (not the Pharmacy) with any questions

NEVER tell a Pt you have not given a particular medication before

MIXING

  1. Obtain a clear/clean space to prepare Elfabrio.

  2. Be sure all the necessary supplies are available to complete infusion from beginning to end.

    • You will need IV start supplies, syringe(s), needle, bag of NS, tubing (gravity tubing or pump tubing if using a pump).

  3. Determine the number of vials needed.

    • Calculate the required Elfabrio vials based on actual body weight in kg

    • Round up to the next whole number of vials needed.

  4. Allow the Elfabrio vials to warm up to room temperature for about 15 – 20 minutes.

  5. Inspect the solution, should be clear and colorless.

    • Remove an equal volume of 0.9% Sodium Chloride from the infusion bag and discard.

  6. Withdraw the required volume of Elfabrio from the vials and inject directly into the NS infusion bag through the port (do not inject into airspace.)

  7. Gently invert the infusion bag to mix.  (Do not shake vigorously.)

ADMINISTRATION

  1. Take baseline vitals and document basic assessment.

  2. Spike the bag of Elfabrio & prime line.

  3. Use an in-line, low protein-binding, 0.2-micron filter during infusion.

  4. Infusion Guideline

    • First 4-6 infusions: Follow infusion rate recommendations.  (See Table 1 and Table 2 Elfabrio pdf)

    • If tolerated, reduce infusion duration every third infusion in 30-minute increments.

    • Minimum recommended infusion duration: 1.5 hours.

  5. Record lot numbers and expiration dates for vials used.

  6. Record vitals every 15 minutes after infusion start for first hour, then hourly AND 5-10 minutes post infusion

  7. After infusion completed, flush the line with 0.9% Sodium Chloride Injection at the same rate as the last part of the Elfabrio infusion.

  8. Disconnect patient, flush line, and remove IV.

    • Place pressure bandage at site.

  9. Clean and dispose of used supplies
