ALWAYSÂ make sure you have everything you need to complete infusion before you begin
ALWAYSÂ confirm your medication dose against the medication label and orders
ALWAYSÂ reach out to us first (not the Pharmacy) with any questions
NEVERÂ tell a Pt you have not given a particular medication before
MIXING
NOTE: Patients receive a loading dose of 3 mg/kg once weekly for the first four weeks. At week 5, the patients start on maintenance dosing so pay close attention to the initiation dose vs the maintenance dose and be sure you are clear on which dose is to be given.
If a dose is missed, administer as soon as possible and then return to usual dosing schedule. DO NOT administer 2 doses on the same day.
Obtain a clear/clean space to prepare HEMLIBRA.
Be sure all the necessary supplies are available to complete the procedure from beginning to end.
You will need a syringe, 18-gauge filter transfer needle and subcutaneous needle and alcohol
Draw up verified dose from vial using syringe and 18-gauge filter transfer needle.
If dose needed is more than 2ml, draw up 2 syringes and give two injections
Change needle to subcutaneous needle provided
ADMINISTRATION
Take baseline vitals and document basic assessment.
Clean injection site with alcohol.
Pinch the skin and insert the needle at 45 or 90 angle.
Release pinched skin and inject at a slow and steady rate.
Withdraw needle at same angle inserted.
Do not rub injection site.
If bruising occurs, apply an ice pack with gentle pressureÂ
Record vitals pre injection and post injection.
Record lot numbers and expiration dates for vials used.
Document dose, site given and how patient tolerated.
Document the number of remaining doses in the home
Clean and dispose of used supplies
