NEVERÂ tell a Pt you have not given a particular medication before
MIXING
Obtain a clear/clean space to prepare the NEXVIAZYME
Remove the required number of NEXVIAZYME vials from refrigerator and allow vials to sit for 30 minutes at room temperature before use
Be sure all the necessary supplies are available to complete infusion from beginning to end
You will need IV start kit, pump, IV tubing with an in-line, low protein-binding, 0.2 micron filter. 5% Dextrose bag, sterile water to reconstitute the vials, Dextrose to flush line when completed
Determine number of NEXVIAZYME vials to be reconstituted
This is based on actual body weight in kg and recommended dose
Number of vials used will be rounded up to the next whole number
Reconstitute each vial by injecting 10 mL of Sterile Water for Injection, down the inside wall of each vial.
Gently tilt and roll each vial to enhance the dissolution process. Do not invert, swirl, or shake vial. Allow solution to become dissolved
Reconstituted solution should be clear, colorless to pale-yellow
Select appropriate size 5% Dextrose Injection infusion bag and prepare by removing a volume equal to required NEXVIAZYME volume and any overfill to achieve a fixed total volume
Slowly withdraw required volume of reconstituted solution from the NEXVIAZYME vial(s)
Gently inject reconstituted NEXVIAZYME into 5% Dextrose Injection bag. Avoid foaming or agitation of infusion bag and avoid introducing air into infusion bag
Gently invert bag to mix solution. Do not shake
ADMINISTRATION
Take baseline vitals and document basic assessment
Begin infusion
Initial recommended rate is 1 mg/kg/hour
Gradually increase the infusion rate every 30 minutes if no signs of infusion-associated reactions
See orders or instructions sent by pharmacy for max rate
Monitor patient for any adverse reactions
Record vitals every 15 minutes for the first hour, then hourly AND 5-10 minutes post infusion
Record lot numbers and expiration dates for vials used
Flush the line with 5% Dextrose when infusion is complete
Disconnect patient, flush line, and remove IV
Place pressure bandage at site
Clean and dispose of used supplies
