MIXING
Obtain a clear/clean space to prepare the
Be sure all the necessary supplies are available to complete infusion from beginning to end. You will need:
Medication, 0.9% sodium chloride injection, USP or 5% dextrose injection, USP for dilution, empty bag, syringe and needle, IV start supplies, IV tubing with in-line filter (pore size of 0.2-1.2 mm), bandage, gauze, alcohol
Withdraw the required volume of OPDIVO and transfer into an empty bag
Dilute OPDIVO with diluent to prepare an infusion with a final concentration ranging from 1 mg/mL to 10 mg/mL. The total volume of infusion must not exceed 160 ml.
For adult/pediatric patients with body weight >_ 40 kg, do not exceed total volume of infusion of 160 ml
For adult/pediatrics patients with body weight <_ 40 kg, total volume of infusion must not exceed 4ml/kg of body weight
Mix dilution solution by gentle inversion
Do not shake
Discard partially used vials or empty vials of OPDIVO
ADMINISTRATION
Take baseline vitals and document basic assessment
Spike the bag with tubing
Infuse over 30 minutes
Monitor patient for any adverse reactions
Record vital signs at baseline, every 15 minutes for the first hour, hourly and with each rate change, AND 5-10 minutes post infusion
Record lot numbers and expiration dates for vials used
Flush the entire IV line at end of infusion
Disconnect patient, and remove IV
Place pressure bandage at site
Clean and dispose of used supplies
