MIXING
Orencia is supplied in a kit. The kit will contain the medication (in powder form in a glass vial) and a silicone-free syringe.Â
Obtain a clear/clean space to prepare the medication. Be sure all the necessary supplies are available to complete infusion from beginning to end. You will need IV start/ port access supplies, silicone free syringe (provided in medication kit), needle, sterile water for reconstitution, 100 ml bag of Normal Saline for dilution, tubing (gravity tubing or pump tubing if using a pump), and a low protein-binding filter (size 0.2 to 1.2micron).Â
Use only the silicone-free syringe provided to withdraw 10 ml of Sterile Water. You will use this same silicone-free syringe to withdraw and inject everything throughout the mixing process.Â
Remove the flip top of the Orencia vial and clean the rubber stopper with alcohol.Â
Inject the 10 ml of Sterile Water into the vial containing the powder. Direct the stream of liquid to the side of the vial. Rotate the vial gently in a swirling motion until the contents are completely dissolved. DO NOT Shake. You can vent the vial with the needle to dissipate any foam if any foaming occurs.Â
Once the appropriate number of vials have been reconstituted, remove the volume of Normal Saline from the 100 ml infusion bag equal to the volume of the total volume of Orencia.Â
Use the same syringe to slowly add the reconstituted Orencia to the infusion bag. Gently mix the contents. DO NOT shake.Â
Spike the IV bag with the proper pump or gravity tubing
ADMINISTRATION
Take baseline vitals and document basic assessment.Â
Infuse Orencia over 30 minutes or per order.
Record vitals every 15 minutes after infusion start AND at infusion end.Â
Record lot numbers and expiration dates for vials used.Â
Disconnect patient, flush line, and remove IV, placing a pressure bandage at site. Clean and dispose of used supplies.
