ULTOMIRIS (IV)

ALWAYS establish IV access prior to mixing any meds

ALWAYS make sure you have everything you need to complete infusion before you begin

ALWAYS confirm your medication dose against the medication label and orders

ALWAYS reach out to us first (not the Pharmacy) with any questions

NEVER tell a Pt you have not given a particular medication before

MIXING

  1. Obtain a clear/clean space to prepare ULTOMIRIS.

  2. Be sure all the necessary supplies are available to complete infusion from beginning to end. 

    • You will need IV start supplies, syringe(s), needle, bag of NS, tubing with  a 0.2 or 0.22 micron filter

  3. Withdraw the calculated volume of ULTOMIRIS from the appropriate number of vials and dilute in an infusion bag using 0.9% Sodium Chloride Injection, USP to a final concentration of:

    • 50 mg/mL for the 3 mL and 11 mL vial sizes

    • 5 mg/mL for the 30 mL vial size. 

  4. Mix bag gently.  Do not shake. Protect from light. 

ADMINISTRATION

  1. Take baseline vitals and document basic assessment.

  2. Spike the bag of 0.9% Sodium Chloride Injection, USP with the ULTOMIRIS

  3. Infuse using a 0.2 or 0.22 micron filter as directed by pharmacy

    • Infuses over 1 hour or less for majority of patients

  4. Record vitals every 15 minutes after infusion start, AND 5-10 minutes post infusion.

  5. Record lot numbers and expiration dates for vials used.

  6. Disconnect patient and flush line.

    • Please note that some orders may require a 1-hour post-infusion monitoring period for signs or symptoms of any infusion-related reaction. Please read your orders and/or any additional instructions provided by the Agency/Care Coordinator carefully.

    • If the 1-hour post infusion period is required, record a set of post monitoring vital signs in the narrative note.

  7. Remove PIV

    • Place pressure bandage at site.

  8. Clean and dispose of used supplies.
