ALWAYS establish IV access prior to mixing any meds

ALWAYS make sure you have everything you need to complete infusion before you begin

ALWAYS confirm your medication dose against the medication label and orders

NEVER tell a Pt you have not given a particular medication before


SCHEDULING

1.    Read orders carefully when scheduling patient. 

2.    Vyvgart has specific orders and is not to be given early.  Use the following site to help schedule next visit.

MIXING


  1. Obtain a clear/clean space to prepare Vyvgart.

  2. Be sure all the necessary supplies are available to complete infusion from beginning to end. 

    ·       You will need IV start supplies, syringe, needle, and sterile water for reconstitution, bag of NS, tubing (gravity tubing or pump tubing if using a pump) and a 0.2 micron in-line filter.

  3. Gently withdraw the calculated dose of VYVGART from the vial(s) with a sterile syringe and needle. Discard any unused portion of the vials. Dilute the withdrawn VYVGART with 0.9% Sodium Chloride Injection, USP to make a total volume of 125 mL for intravenous infusion.

  4. Gently invert the infusion bag containing the diluted VYVGART without shaking to ensure thorough mixing of the product and the diluent.

  5. Administer immediately after dilution and complete the infusion within 4 hours of dilution.

ADMINISTRATION

  1. Take baseline vitals and document basic assessment.

  2. Infuse the total 125 mL of diluted solution intravenously over one hour via a 0.2 micron in-line filter. 

  3. Monitor patients during administration and for 1 hour thereafter for clinical signs and symptoms of hypersensitivity reactions.

  4. Record vitals every 15 minutes for the 1st hour, then hourly thereafter AND 5-10 minutes post infusion.

  5. Record lot numbers and expiration dates for vials used.

  6. After administration of VYVGART, flush the entire line with 0.9% Sodium Chloride Injection, USP

  7. Disconnect patient and flush line.

  8. Monitor patient for 1 hour post and document vital signs in narrative.

  9. Remove IV. Place pressure bandage at site.

  10. Clean and dispose of used supplies.
