ZEMAIRA (IV)

alpha1-proteinase inhibitor (human)

MIXING

  1. Obtain a clear/clean space to prepare the

  2. Be sure all the necessary supplies are available to complete infusion from beginning to end. 

    • You will need IV start supplies, alcohol, syringe, IV administration set with a 5-micron infusion filter

  3. Reconstitute the ZEMAIRA with sterile water using the Mix2Vial transfer set that is provided

    • ZEMAIRA may be ordered with a 10-15% deviation allowance and if so, it will be noted in the order

    • Verify the dose is within the ordered range

  4. Gently swirl the ZEMAIRA vial until the powder is completely dissolved. 

    • DO NOT SHAKE

  5. Reconstitute the required number of vials

    • The solution should be clear colorless to slightly yellow, and free from visible particles

  6. If more than 1 vial of ZEMAIRA is needed to achieve the required dose, transfer the reconstituted solution from the vials into the administration container (e.g., empty intravenous bag or glass vial)

ADMINISTRATION

  1. Take baseline vitals and document basic assessment.

  2. Infuse at a rate of approximately 0.08 ml/kg/min

    • The recommended dosage of 60 mg/kg body weight will take approximately 15 minutes to infuse

  3. Record vitals every 15 minutes AND 5-10 minutes post infusion.

  4. Record lot numbers and expiration dates for vials used.

  5. Disconnect patient, flush line, and remove IV.

  6. Place pressure bandage at site. 

  7. Clean and dispose of used supplies.

  8. DOCUMENT the EXACT dose given (not the dose ordered if there is a dosage range)
