• Teachable medications (generally, with some exceptions)

• A syringe driver pump (Freedom 60) is most often used for these infusions

• Three teach visits is standard, but up to 5 visits is acceptable

• Frequency for is generally weekly, sometimes EO week and rarely monthly.

​Education/Teaching visits for these medications are generally as follows:

​

Visit 1: RN does the infusion start to finish, verbally explaining each step to patient/caregiver and acknowledging their understanding.

Visit 2: patient/caregiver completes the infusion with verbal assistance from RN as needed. RN can leave once infusion is started and patient/caregiver demonstrates comfort and ability to complete infusion on their own. Nurse may choose or be asked to stay for the entire infusion to ensure comfort/competence.

Visit 3: Patient/Caregiver completes infusion while RN watches to ensure everything is done correctly. RN can leave once patient is up and running.

Visits 4-5: Additional teach visits only as needed or desired by the patient/caregiver. Goal is to teach patient/caregiver to be independent with infusions. If this is not achieved in 5 visits, please let us know so we can communicate with the patient's pharmacy and MD.

​Hyqvia

​

• Teachable medication, but can be approved for RN infusion

• The CADD pump is most often used for these infusions

• Frequency is monthly, but often includes ramp-up dosing

• Three teach visits is standard (non ramp-up), but up to 5 visits is acceptable and common with ramp-up dosing

​Unless otherwise ordered, Hyqvia patients ramp-up their dosing as follows:

• Week 1: 25% of total monthly dose

• Week 2: 50% of total monthly dose

• Week 4: 75% of total monthly dose

• Week 7: full ordered dose

Teaching Hyqvia ramp-up patients is generally 5 visits, unless the patient/caregiver demonstrates a high-level of competence and can be taught during the ramp-up period.

 

Teach visits with a ramp-up are generally completed as follows:

Visits 1, 2 & 3 (Weeks 1, 2, 4): RN does the infusion start to finish, verbally explaining each step to patient/caregiver and acknowledging their understanding.

Visit 4 (Week 7): patient/caregiver completes the infusion with verbal assistance from RN as needed.
