On Day 1, RN will:
Complete the SOC/Initial Assessment Paperwork with the patient.
Establish a peripheral IV (Educate patient on signs and systems of IV infiltration and ensure they have the agency contact information in the event they need assistance).
Teach patient how to independently administer ordered Hydration and prn Antiemetic. Hydration will run via gravity/dial-a-flo-tubing. RN may leave once the patient demonstrates and verbalizes understanding and feels confident to self-administer on Day 2 and Day 3.
Document in narrative that patient verbalized understanding, is independent, and requires no further teaching. If patient needs additional teaching, document why additional teaching is needed and notify agency. Agency will reach out to Pharmacy and request additional teach.
On Day 3, RN will:
Visit patient after the last Hydration infusion has been completed. RN will complete a follow-up assessment. A standard note is sufficient for this visit.
Complete note as usual and ask and document the following questions:
How are you doing?
Have you had to use the IV push anti-emetics? If so, how often?
Have you had any N/V? If so, how often?
If patient answers Yes to last two questions or reports any other concerning symptoms, report to agency while still in the home. Agency will report to the Pharmacy, UNC Homecare Specialists, and to Provider. The Provider may decide to place an order for continuation of therapy. Pharmacy will handle filling the order and getting the additional Hydration/Anti-Emetics to the patient.
