MIXING
NOTE: Patients/Caregivers are usually taught.
Obtain a clear/clean space to prepare the lock therapy.
Be sure all the necessary supplies are available to complete the visit from beginning to end.Â
You will need alcohol, normal saline flush, lock therapy ordered, and empty syringe
Prepare the lock therapy.
Types of lock therapy:
Ethanol
Antibiotics (Vancomycin, Gentamicin are two most common)
KiteLock 4%
ADMINSTRATION
NOTE:Â If more than one lumen on the catheter, you will need 1 dose for each lumen.
Take baseline vitals and document basic assessment.
Clamp the catheter.
Scrub the hub with alcohol.
Flush the line with ordered amount of NS and clamp as infusing the last of the NS.
Scrub the hub with alcohol.
Unclamp and infuse the ordered amount of the lock, clamp line as infusing the last of the lock therapy.
Remove syringe and place disinfection cap (if applicable).
Allow the lock to dwell for the required amount of time.
After the prescribed amount of time scrub the hub, open the clamp and pull back 2 ml of fluid. This 2 ml will be a mixture of the lock and blood. Discard this syringe. Clamp the line.
Scrub the hub and connect a new NS syringe, unclamp and push ordered amount of NS into the line. Clamp the line.
Note Heparin is NOT compatible with Ethanol or Gentamicin. You must flush with saline between each medicine.
Clean and dispose of used supplies.
